Our office is in the process of changing to an electronic medical record (EMR), a computer-based method to store medical records and charts. Currently our new GYN patients will be required to complete the online history. Eventually each patient will be required to fill in a medical history online before her visit. You will receive a unique username and password from our office to log in. If you do not receive this information, please contact our office. Prior to your scheduled appointment, we would like for you to visit https://patients.digichart.com (or click on the digiChart icon below) to fill out a detailed medical history. This should take 30 minutes to complete and can be done from any computer with Internet access. By completing this history ahead of time, you will save time at your next visit. The information you provide is secure and protected, and complies with all federal and HIPAA regulations. If you are unable to complete your medical history prior to your appointment, please arrive at least 30 minutes before your scheduled appointment to complete this information at one of our computers. Remember, there could be a wait for your turn at the computer which could greatly increase your wait to see the provider or you may have to reschedule your appointment. Our desire in adopting an EMR is to provide a better experience for our patients, allowing us to provide the best, most timely, and up-to-date medical care. We greatly appreciate your patience and understanding as we adopt these changes.
