Patient Portal Instructions
If this is your first visit since June 1, 2016, you will need to complete the online medical form on our new patient portal prior to your visit, or your appointment may be rescheduled.
After logging into the Patient Portal go to My Health tab then click on Medical Forms and click on the Patient History to complete.
If unable to complete Online Medical Form please arrive for your appointment 30 minutes early.
Please give 48 hrs notice to cancel or reschedule this appointment. Please remember to bring your photo ID and either your insurance card or copy of your card. We cannot accept digital insurance cards. We look forward to your visit.
