Patient Portal Instructions
If this is your first visit since June 1, 2016, you will need to complete the online medical form in our new patient portal prior to your visit, or your appointment may be rescheduled.
7 days prior to your scheduled appointment you can sign into the Patient Portal.
Go to Home - Appointments - Click on the scheduled appointment – click on Check-In button –You are asked to confirm your appointment and write any questions you may have to discuss with doctor. Continue- Review your information – Save and Continue – Online Health history form needs to be completed. Enter your Medications, Allergies, Immunizations, Medical History, Surgical History, Social History, Family History and Obstetrics/Gynecology information, at the end make sure you save and it should tell you that you have completed.
If you are an established patient you will need to log into the Patients Portal and review all information and select No Changes or Update to update your information.
If unable to complete Online Medical Form please arrive for your appointment 30 minutes early.
Please give 48 hrs notice to cancel or reschedule this appointment. Please remember to bring your photo ID and either your insurance card or copy of your card. We cannot accept digital insurance cards. We look forward to your visit.
